Zusammenfassung

Kleine Pilot-Fallserie (n=3 Patienten mit therapierefraktärer Epilepsie bei primärem Hirntumor) unter pharmazeutischem Cannabidiol (CBD, Epidiolex). Zwei von drei Patienten zeigten eine verringerte Anfallshäufigkeit, alle drei eine geringere Anfallsschwere. Vorläufiges Signal, wegen sehr kleiner Fallzahl und fehlender Kontrollgruppe nicht belastbar; laut Autoren nur Anlass für weitere Studien.

Abstract
Epilepsy, commonly encountered by patients with brain tumors, is often refractory to standard therapies. Our aim was to examine the safety and efficacy of pharmaceutical grade cannabidiol (CBD; Epidiolex; Greenwich Biosciences) in those patients with epilepsy with concomitant tumors enrolled in The University of Alabama at Birmingham CBD Program (NCT02700412 and NCT02695537). Of the three patients with refractory seizures and a history of a primary brain tumor, two had improvement in seizure frequency and all three had improvement in seizure severity. These pilot results suggest that CBD should be further studied for the treatment of brain tumor-related epilepsy.

Effect of Cannabidiol on Drop Seizures in the Lennox-Gastaut Syndrome.

Multizentrische Phase-III-RCT bei Lennox-Gastaut-Syndrom (n=225, Alter 2-55 Jahre, 30 Zentren), Cannabidiol 20 mg/kg vs. 10 mg/kg vs. Placebo (14 Wochen). Baseline: median 85 Drop-Seizures/28 Tage. Mediane Reduktion der Drop-Seizure-Frequenz: 41,9% (20 mg/kg, p=0,005 vs. Placebo), 37,2% (10 mg/kg, p=0,002 vs. Placebo), 17,2% (Placebo). Häufigste Nebenwirkungen: Somnolenz, verminderter Appetit, Diarrhö (dosisabhängig); 6 Patienten (20 mg/kg-Gruppe) und 1 Patient (10 mg/kg-Gruppe) brachen wegen Nebenwirkungen ab.
